These health questions are run by Dr Nathan So, anaesthetist, to plan a safe anaesthetic for you.

What we collect

Your name, date of birth, mobile number, surgery details, and your answers about your health, medicines and lifestyle, including any photos you add.

Why

To check you are safe for your anaesthetic, to decide whether Dr So should contact you before surgery, and to give you advice to prepare. Your answers form part of your medical record.

Who sees it

Dr So and staff who help him care for you. We do not sell your information or use it for marketing. We share it with others involved in your care only where the law allows.

Where it is kept

On secure servers in Sydney, Australia. Access is limited to Dr So. Records are kept for as long as NSW law requires for medical records (usually 7 years for adults, and until age 25 for children).

Your rights

You can ask to see or correct your information. Contact Dr So's rooms, Eastern Suburbs Anaesthetics, on 02 9389 6600. If you are unhappy with how we handled your information, you can complain to the Office of the Australian Information Commissioner (oaic.gov.au) or the NSW Information and Privacy Commission (ipc.nsw.gov.au).
